    Nutrition Support Dietitians are responsible for assisting in the safe and effective transition of patients to home enteral nutrition therapy.  This position is responsible for handling patient and caregiver calls in a fast-paced environment, while maintaining emphasis on the nutritional care and well-being of our patients.  This includes assessing patients’ nutritional risks, developing care plans to meet nutritional needs of patients, communicating with the patient’s broader care team, and providing counseling and education of patients, family members and caregivers. 



    • Functions as part of a team which administers and implements the Company’s enteral nutrition program and related therapies and services.
    • Telephonically screens and assesses specific patient populations as appropriate to verify nutritional needs in accordance with Company policy and practice standards; as appropriate, communicates findings and recommendations to physician and other members of the patient’s health care team.
    • Reviews necessary information collected and patient nutrition and caloric needs calculated during intake process, to continue nutrition support in the home in accordance with Company policy and practice standards.
    • Participates in the development, documentation, and implementation of nutrition care plans and provides nutrition education and guidance to patients, caregivers and clinical staff.
    • Communicates patient learning needs and findings to health care team members, including other clinicians, customer service staff, home health agency and physician/referral source, as appropriate.
    • Maintains current and appropriate levels of communication with physician, home health agency and/or referrer regarding patient status and appropriateness of current prescription.
    • On an on-going basis, monitors patient progress toward collaborative home nutrition goals.
    • Monitors and evaluates outcomes of nutrition intervention.
    • Monitors and evaluates patient competence and compliance with all self-care, including procedures and infection control; reeducates as necessary, and documents/communicates the need for reeducation. 
    • Appropriately documents patient care activity, dietitian services provided, and all communication with patient and physician.
    • Reviews documentation for accuracy, completeness and compliance with Company policy and procedure for electronic and paper patient clinical Dietitian record entries. 
    • Conducts ongoing chart review per policy. 
    • Forwards required documentation to physician, payors, and referral sources within policy or contractually stated timelines.
    • Facilitates the transition of enteral support to oral diet, as appropriate.
    • Collaborates or assists with transition of care when insurance coverage necessitates a change in home care provider to ensure continuity of care.
    • Works with the revenue management department to ensure prescriptions and clinical chart documentation contain all necessary information for prompt payment.
    • Participates in education and training conferences on home nutrition care. 
    • Participates in training sessions of sales personnel marketing the nutrition program.  May assist sales personnel in clinical aspects of marketing home nutrition programs.
    • Keeps abreast of evidence-based recommendations and guidelines for nutrition.

    Minimum Required Qualifications



    Education and/or Experience

    • B.S. degree in Food/Nutrition or Dietetics is required
    • At least one year related experience is required


    Certificates, Licenses, Registrations or Professional Designations

    • Registered Dietitian with the Commission on Dietetic Registration of the American Dietetic Association
    • Maintains required license and/or certification in accordance with state licensure requirements and CDR, as applicable
    • Must get licensed by the Commission on Dietetic Registration in 2 or more additional states after the date of hire.



    • Experience with Microsoft Office


    Language Skills

    • English (reading, writing, verbal)

    • A.S.P.E.N. certification is preferred
